There will be NO CHARGE* to any uninsured family members (parents, siblings, etc.) who are dealing with grief & loss issues over the recent (within one year) employment-related death of a military service member, fire fighter, or police officer (*applies to in office services only).
All Service Fees are billed/charged per fifteen minutes and apply to the nearest full quarter-hour. The fee period is from engagement to departure.
Contracted rates apply only if the therapist is/was - at the time of service - contracted as a preferred provider with your medical insurance carrier or Employee Assistance Program (this means there is a separate fee agreement with your insurance or EAP - and the fees are by prearranged contract with that agency).
