Fees are $22.50 per quarter hour (15 minutes) for written reports/services that are designed to be released outside of this agency.

Most insurances do not cover many written report services.

There are no charges for written reports to contracted third-party payers or referring physicians. Written reports include attendance/treatment reports, diagnostic assessments, evaluation summaries, progress reports, and etc. (such as adoption agency reports, agency assessment and recommendation reports, court reports, DCFS reports, employer reports, and the like).
