No-shows:
There is a no-show (failed to call, forgot, changed my mind, gave no notice of cancellation) fee of $30.00.
There is never a charge for your cancellation made with more than 24 hours notice.
We want your appointments to be convenient for you.
We work hard to make your times available - please respect this by showing up!
Prior notice of any scheduling change is very much appreciated.
But, understandably - things happen, illness occurs, or emergencies appear.
We will do our best to notify you as soon as possible of any schedule change of ours!
We give one courtesy "get-out-of-unkept-appointment-fee-free" card per intake or per ten visits (With this we will reverse the no-show fee on exchange of your card, or at your request - in lieu of the card). No contact from you means no reversal of the charges. The get-out-of-fee card is not transferable.
No-show fees are completely waived on written verification of medical emergency (physician’s notes or a copy of hospital ER/admission notes) if provided within 30 days.
Cancellations may be made by telephone (person-to-person, or voice mail), e-mail, fax, or a telegram..
