Cancelled Appointments:
There is no charge for your in office visit cancellation if made with more than 24 hours notice.
There is a "same day cancellation" fee of $30.00 for appointments canceled the same day, prior to the appointment.
There is a "no show" fee of $40.00 for missed appointments (failed to cancel, forgot, changed mind).
There is a "cancellation" fee of $20.00 for appointments canceled within 24 hours, but done the day before the appointment.
Cancellation fees may be waived with written verification of a medical emergency (copy of physician or hospital admission notes - dated within 24 hours of your cancellation) if provided before we bill you.
We want your appointments to be convenient for you.
We work hard to make your times available - please respect this by showing up!
Prior notice of any scheduling change is very much appreciated.
But, understandably - things happen, illness occurs, or emergencies appear.
We will do our best to notify you as soon as possible of any schedule change of ours!
Cancellations may be made by telephone (person-to-person, or voice mail), e-mail, or a telegram.
